- Parent and student IRS 1040 forms
- Financial aid award letter from the college
- Applications must be returned to us by April 17
- Families should plan tax work to meet this deadline

Click here here for an undergraduate application form. There is one application for all undergraduate applicants and scholarships. You must be accepted to or currently enrolled in a certificate program, two-year or four-year college degree program.
Click here for a graduate application form. There is only one scholarship which is available for either an undergraduate or graduate student.
Note: There are three scholarships with specific criteria.
- The George O. Smith Scholarship requires that a student major in a physical science or engineering AND submit a copy of the student's birth certificate with the application. The student's mother MUST have been a Lexington resident at the time of the applicant's birth. Examples of qualifying majors: astronomy, chemistry, earth science, engineering, geology, meteorology, oceanology or physics.
- The Emily and Frank Smiddy Scholarship requires that a student intends to study in STEM fields (science, technology, engineering or math) upon entering their first year of college.
- The Phyllis E. Richardson Scholarship requires a student graduated from Lexington High School and attended a Lexington Middle School. Student must study Teaching, Nursing or STEM.
